To facilitate being seen at the time of your appointment, we would appreciate it if you would review and complete the Patient Registration Forms, Financial Policy, Privacy and HIPAA Consent forms listed under Online Forms before your arrival. Please remember to bring a current photo ID and any dental insurance cards that may apply to you or your family so we can scan a copy of them for your records. Please arrive 15 minutes prior to your appointment time in order for our office to complete a complementary insurance benefit check prior to your appointment.
We thank you in advance for honoring your appointment. If you are unable to make the appointment you have scheduled with us, please notify us at least 24 hours in advance. If an appointment is cancelled, rescheduled or missed within a 24 hour period, a missed appointment fee may be applied to your account.
Locust Lane Dental Group will help you receive maximum benefits from your insurance policy. Though we may not participate with your particular dental insurance provider, we still process all dental insurance policies. We currently participate with Delta Dental, United Concordia, Blue Cross Dental and UPMC Pinnacle. Please inform our front desk staff of all dental insurance plans you are covered by, so we can submit your claims properly. Insurance plans vary considerably and we cannot guarantee what part of our services will or will not be covered by your insurance plan. Our office can only provide an ESTIMATE of coverage. You are responsible for any services not covered by your insurance plan. You are also responsible for being aware of your insurance contract maximum and covered benefits. The office is not responsible for keeping track of the amount of insurance money you have utilized for the contract year. You are ultimately responsible for all of your insurance co-pays at the time of your appointment. We will expect you to pay any outstanding balances not covered by your insurance before we provide any future treatment. Please contact your insurance carrier or your employer’s human resources department if you have any questions or concerns we cannot answer.
